To replace your Priority Health (Medical) insurance card, you have 2 options:

1. Go digital! You can view your digital ID card by logging in to member.priorityhealth.com and clicking on the “ID CARD” button on your dashboard. This feature allows you to print or fax the insurance card to providers directly from your device.
    
2. Request a physical replacement card. You can also “Request a Card” from this menu. Requests take up to 7 business days and will be mailed to your home address on file.

To replace your ASR Health Benefits (Vision/Dental) insurance card, you have 2 options:

1. Go digital! You can view your digital ID card by logging in to www.asrhealthbenefits.com and clicking on “Change My Info” → “Member ID Card Request”. At the top of the page it has an option to click on “Download a PDF copy of your current ID Card".
    
2. Request a physical replacement card. In the same location as mentioned above, you can request a new physical card and the number of ID cards needed. Replacement cards will be mailed to Berrien RESA and given to the employee at the office when they arrive.

Employees covered under a collective bargaining agreement need to check the contract for special leave provisions.  Non-bargaining unit members may request special leaves with or without pay.  These leaves must be requested through, and approved by the Director of Human Resources.  Leaves without pay will also be without fringe benefits (including, but not limited to: insurance, experience credit, holidays, vacation, other paid leave entitlements) and are not guaranteed, because your presence at work, as scheduled, is necessary to provide quality educational services to our students and families.

A calendar year is January 1 through December 31.

A fiscal year is July 1 to June 30.

Berrien RESA follows a fiscal year calendar. This calendar dictates our budget cycle, insurance cycle and school calendars.

To update your State of Michigan W-4 Form, contact Jordan Kugel or Joanna Davis in the HR Department. They can provide you with a new paper copy form to fill out.

Updating your Federal W-4 Form is done through TalentED. Again, reach out to Jordan or Joanna and they will assign this task to you through the TalentED portal to be completed electronically.

Please contact Jordan Kugel right away. She will be able to print a new badge for you (and take a new photo, if you would like one!).

As for your key fob, please let Lisa Strzyzykowski in the Facilities & Operations Office know. She will reassign you a new key fob. There is a small fee to replace lost fobs.

We understand that losing a family member is an incredibly difficult and emotional time. Please know that we are here to support you during this challenging period.

If an immediate family member passes away, we encourage you to notify the HR Office as soon as possible. You are eligible for up to 5 bereavement days to allow you time to grieve and make necessary arrangements.

Immediate family members include:

• Your current spouse
• Your parents, stepparents, and in-laws
• Your siblings
• Your children, stepchildren and their spouses
• Your grandparents and great-grandparents

While bereavement leave is only provided for immediate family, we understand you may need time off to mourn other loved ones as well. You are welcome to use any available personal time, which can be discussed with your supervisor.

If you would like the HR team to share the obituary information with our Berrien RESA staff, please contact us with the request and a link to the obituary. We want to ensure your colleagues are aware and can offer their condolences during this difficult time.

Log into your ASR Health Benefits Portal. (If you've never logged in to your account before, login using the information on your ASR insurance card.

Once logged in, Click on “My Plan Coverage” and then “Flexible Spending” from the menu. From there, you should be able to see claim information as well as available balances.

While HR is always here to provide support and guidance on a variety of matters, unfortunately we are unable to provide tax advice or assistance with personal tax-related issues. Tax laws and financial situations can be complex and vary from person to person, so it is important to seek advice from a qualified tax professional or financial planner for accurate and personalized guidance.

To find out what you are currently claiming, log into Skyward/Qmlativ and go to “Employee Access” → "Payroll” → "Tax Information".

In this menu, you should be able to see what status and allowances you are claiming. As a general rule of thumb, the more exemptions you claim, the less taxes will be taken out of each paycheck.

If you need to make a change to your Federal or State W-4 form, please reach out to Joanna Davis in HR. She will then assign a new W-4 form in the TalentEd platform and update your withholdings in our systems.
